In about 1% of cases, it's due to broken ACLs on the WSUS server (or insufficient disk space), but on a new server, and assuming you have not "customized" anything, then it's likely a proxy issue. Either the proxy server is blocking EXE/CAB/TXT downloads across port 80, or more likely, particularly with older devices/software, the proxy server is not properly configured, or not capable of, supporting the HTTP 1.1 protocol specification's Range Protocol Headers feature, which is required to download files with BITS in the background. In scenarios where ALL downloads failed, it's almost always a proxy server at the core of the issue. Look in the Application Event Log for EventID 364 which will identify exactly why the downloads failed. Click Retry Download to start the download again."
